Referral

Availing of our services is easy. Anyone can make the referral to Americare Hospice & Palliative Care. We just need basic information on the patient to get started with the process. We can receive this information by calling us at (480) 726-7773, or by filling out the simple Contact form on the right hand side of this page.  A hospice nurse will then speak to you about hospice services and how to enroll if hospice is right for you.
