Signing Up For Hospice

A referral to hospice can come from a patient, a caregiver, relatives and friends, a physician, a hospital discharge planner, or a nursing facility social worker.

A hospice consultation to discuss the philosophy, criteria, and services of Americare Hospice & Palliative Care is done to inform the patient and caregivers and see if hospice is right for them. The patient’s physician gives confirmation of the patient’s life-limiting illness.    Once enrolled under hospice, a nurse will come to do the admission visit and initiate the plan of care with the patient, family, and team.
